Baby Baptism Ceremony
BAPTISM

Arrangements should be made at least one month prior to baptism.

Baptism is celebrated on the second and fourth Saturdays ( without the Holy Mass)

and Sundays (during or right after the Chosen Holy Mass). 

One preparation class is required to both parents and both godparents.

The class will meet on the first Wednesday of the month at 7:00 p.m. in the Rectory Hall in English. Spanish class meets on the second Wednesday of the month at 7:00 p.m. in the Rectory.

Polish class is scheduled by appointment.

                                                         REQUIREMENTS FOR BAPTISM

  • Baptismal Record Information Form(click at PDF document above)

  • Birth Certificate of the child

  • Baptism Prep Class for both parents and godparents

  • Letter of eligibility from one of the godparents provided by the godparent's parish

  • Donation

                                                         REQUIREMENTS FOR GODPARENTS

​

According to the Code of Cannon Law (chapter IV:872,873,874), a candidate for a Godparent must meet the following conditions:

  • To be appointed by the parents of the baptized person and have the required qualifications and intentions to accomplish this task

  • Be over 16 years old

  • Be a confirmed Catholic leading a life in accordance with the faith

  • Not to be a mother or father of the baptized​

One of Godparents is required to provide a letter of eligibility from their parish​

The task of the Godparents is to support the parents in the Christian upbringing of the child in accordance with the teachings of the Catholic Church, and to faithfully fulfill the obligations of the Godparent undertaken at baptism

Religious Objects
MARRIAGE

An appointment should be made with one of the priests or deacons at least six months prior to the wedding. This should be done before a date or a banquet hall is reserved.

                                                         REQUIREMENTS FOR  WEDDING

​

  • Certificate of Baptism from Bride and Groom

  • Certificate of Confirmation from Bride and Groom (not required by Cannon Law, but recommended)

  • Statement from 2 witnesses from the Bride side, given to the pastor in person

  • Statement from 2 witnesses from the Groom side, given to the pastor in person

  • Marriage statement of Bride and Groom given to the pastor

  • Making arrangements regarding date, music, rehersal

  • Additional documents are required in special circumstances

SACRAMENT OF RECONCILATION

Sacrament of Reconciliation is offered every Saturday from 3:00 to 3:45 p.m. You may also call the rectory for an appointment.

Holding Hands
SACRAMENT OF THE SICK

Call the Rectory at 773-637-6565 for a priest. Please do not wait until the person is unconscious or near death.

Communion
COMMUNION CALLS

If you would like to receive Communion, call the rectory at 773-637-6565 to make arrangements.
